What Is Situational Awareness in the NHS?

An angry patient threatens a nurse with his walking crutch

In the NHS, situational awareness means knowing what is happening around you, understanding what it means for your patient or team, and thinking ahead to what might happen next. It is one of the key non-technical skills in NHS human factors training, alongside communication, teamwork and decision-making.

In clinical settings it is most often linked to patient safety. That means spotting the early signs that a patient is deteriorating, or noticing when a team has become fixated on one problem. The case of Elaine Bromiley, who died in 2005 after clinicians became focused on repeated attempts to secure her airway, is widely used in NHS training to show how quickly situational awareness can be lost under pressure.

The same skill matters for staff safety. NHS staff regularly face aggression from patients and relatives. Noticing the warning signs early gives you the chance to de-escalate before it becomes an incident. Those signs might be a change in tone, agitation building in a waiting area, or someone moving between you and the door.

That is the focus of our Managing Challenging & Aggressive Behaviour Training for Healthcare. It applies the wider meaning of situational awareness to realistic healthcare scenarios.
